Client onboarding questionnaire template

This five-step client onboarding questionnaire asks what the business sells, its main goal, its marketing channels and the accounts you can access, followed by the contact details. You can clone this template into your Growform account and edit any question.

What the form asks, step by step

An onboarding questionnaire should give your team enough to plan the first week with a new client. This one opens with a written answer and then moves to buttons.

  1. Step 1: “What does your business sell, and to whom?”

The client describes the offer and the audience in their own words.

  2. Step 2: “What is your main goal for the next 90 days?”

A single answer tells your team which result the client will judge you on.

  3. Step 3: “Which marketing channels do you use today?”

More than one channel can be selected, which shows where work already exists.

  4. Step 4: “Which accounts can you give us access to this week?”

Your team learns which logins to request before the kickoff meeting.

  5. Step 5: “Who should we work with day to day?”

Name, email and phone come last.

  6. After submitting

The closing screen confirms that you have what you need to get started, and you can rewrite it to give the date of the kickoff call.

Who it’s for

This template is for agencies, consultants and freelancers who have just signed a client and need the basics before work begins. Sent with the welcome email, it replaces the long document that clients leave unanswered for a week. Each submission gives you the offer, the 90-day goal, the channels already running and the accounts the client can open to you. Some teams call it a client onboarding form and others a new client intake form, and the template serves both uses.

If the client has not signed yet, the marketing questionnaire for clients asks about budget and timing instead.

Why this structure converts

  • The only written question comes first, when the client is most willing to type.
  • Goals, channels and access are answered with buttons, so the remaining steps take very little effort.
  • The access question asks only which accounts are available, which keeps passwords out of the form.
  • The contact step asks for the day-to-day contact, who is often a different person from the buyer.

Make it yours

  • Add a budget step

If you manage paid media, add a step with monthly budget bands after the channels question so the team can plan spend before the kickoff.

  • Name your own services

Replace the five goals with the outcomes your agency sells, such as booked appointments or qualified demos, so that each answer maps to a service.

  • Ask about stakeholders

Add a short text question for the person who approves work. Approval delays are easier to prevent when you know that name from the first day.

Frequently asked questions

What should a client onboarding questionnaire include?

It should cover what the client sells, the result they want, the marketing already in place and the access your team needs. This template asks those four things and then the name of the day-to-day contact.

How long should an onboarding questionnaire be?

It should be short enough to complete in one sitting. Four questions and a contact step make a sensible base, and you can add steps for the details your service depends on.

Can clients share passwords through the form?

The template does not ask for passwords. It asks which accounts the client can share this week, and your team can then request access through the user settings of each platform.

Where do the onboarding answers go?

Each submission arrives by email by default. You can also send it to HubSpot, ClickUp, Asana or a Google Sheet, so the account team sees it where the project is managed.
