Quote request form template

This five-step quote request form asks the service, home or business, the timing and the job details, followed by the contact details. You can clone this template straight into your Growform account and rename the services.

What the form asks, step by step

Three button questions describe the request, and a written answer adds the detail. The contact step then asks where the quote should be sent.

  1. Step 1: “What do you need a quote for?”

Installation, repair and maintenance are priced differently and often handled by different people.

  2. Step 2: “Is this for a home or a business?”

Commercial work usually means a site visit and a larger quote.

  3. Step 3: “When do you need the work done?”

Urgent jobs can be called first, and price comparisons can be answered by email.

  4. Step 4: “What should we know about the job?”

An open text field lets the customer describe the job in their own words.

  5. Step 5: “Where should we send your quote?”

Name, email and phone come last.

  6. After submitting

The closing screen thanks the customer and says that the quote will follow soon, and you can replace that with your own response time.

Who it’s for

This quote form template suits any business that prices work on request, from a contractor or an installer to a repair shop or a consultancy. It is general by design, so you can rename the services on step one and use it as a request a quote form template for almost any trade. Each submission tells you what is needed, who it is for and how soon, which is enough to prepare a price or to arrange a visit. Some trades call the same document an estimate form, and the template serves both purposes.

If you work in a single trade, the roofing and plumbing service templates are written for those jobs.

Why this structure converts

  • The first three questions take one tap each, so the written description comes after the customer has already started.
  • Timing is asked before the details, and its last answer identifies people who are only comparing prices.
  • The contact step asks where to send the quote, so giving an email address has an obvious purpose.

Make it yours

  • Rename the services

Replace installation, repair and maintenance on step one with the services you sell, and keep “Something else” so that unusual requests still reach you.

  • Add a ZIP code step

Insert a ZIP code question before the contact details if you only cover certain areas or if travel distance changes your price.

  • Guide the written answer

Add helper text under the details question suggesting what to mention, such as sizes, quantities or access, so the descriptions you receive are easier to price.

Frequently asked questions

Which questions belong on a quote form?

A quote form needs the service, the type of customer, the timing and a description of the job. This template asks those four things in that order and requests contact details at the end.

Is a quote form the same as an estimate form?

In practice, yes. Both collect enough detail to price a job, and you can change the wording on the first and last steps if your trade says estimate.

Does the form send the quote automatically?

No. The template collects the request and you reply with the price. Each request is emailed to you, and it can also be sent to HubSpot, Pipedrive or a Google Sheet.
