Inquiry form template
This four-step inquiry form asks the topic, whether the person is an existing customer and how you can help, followed by the contact details. You can clone this template straight into your Growform account and rename the topics.
What the form asks, step by step
The inquiry form template sorts each message before it is written. Two button questions come first, followed by the message and the contact step.
-
Step 1: “What is your inquiry about?”
The topic decides which team should receive and answer the message.
-
Step 2: “Are you an existing customer?”
Existing customers can be matched to their account before anyone replies.
-
Step 3: “How can we help?”
The message is written after the topic, so it arrives with context.
-
Step 4: “How shall we reply?”
Name, email and phone come last.
-
After submitting
The sender is thanked for the inquiry and told that the right person will reply soon.
Who it’s for
This template is for companies that receive every kind of message through one contact page. It replaces a single open text box with a short sequence that records the topic and whether the sender is already a customer. A sales inquiry form and a support request usually need different people, and the first answer tells you which one you are reading. The template sends every submission to the same destination, and the topic is included so that you can sort or assign messages in your help desk or CRM.
Why this structure converts
- Choosing a topic takes one tap and shows the sender that the message will reach the right team.
- The customer question on step two removes the need to ask about an account in the first reply.
- The contact step asks how you should reply, which explains why the details are needed.

Use your own topics
Rename the five topics on step one to match your departments, and keep a general option for messages that fit none of them.
-
Add an order number field
If most messages come from customers, add an order or account number field to the message step to shorten the first reply.
-
State your reply time
Rewrite the thank-you message to state your normal response time so that senders know when to expect an answer.

Frequently asked questions
Is an enquiry form different from an inquiry form?
No. Enquiry is the British spelling of the same word. If your customers are in the United Kingdom, you can retitle the template as a customer enquiry form by editing the wording on each step.
Does the template route inquiries to different teams?
Not by itself. Every inquiry goes to the same inbox or integration with the topic attached, and you can use that answer to assign the message in tools such as Zendesk, Help Scout or HubSpot.
What does the contact step ask for?
It asks for a name, an email address and a phone number. You can remove the phone field if you only reply by email.
