Tax client intake form template

This five-step tax client intake form asks the filing status, the types of income, who prepared last year’s return and any notes, then the contact details. You can clone this template into your Growform account and add further questions.

What the form asks, step by step

The questions tell a preparer how complex the return is likely to be before any documents are exchanged. None of them asks for a figure.

  1. Step 1: “What is your filing status?”

Filing status is the first fact a preparer needs to scope the return.

  2. Step 2: “Which types of income did you have this year?”

Several income types can be selected, and each one adds work to the return.

  3. Step 3: “Who prepared last year’s return?”

The answer shows whether prior records are with the client or another firm.

  4. Step 4: “Is there anything else we should know about your taxes?”

An open field lets the client mention notices, deadlines or changes in circumstances.

  5. Step 5: “How shall we contact you?”

Name, email and phone come last.

  6. After submitting

Clients finish on the default thank-you message, which is a good place to list the documents to gather.

Who it’s for

Independent tax preparers, enrolled agents and accounting practices can use this template to take on new clients during tax season. As a tax intake form it records filing status, the kinds of income the client had, who prepared the previous return and any notes the client wants to add. That is enough to estimate the work involved and book an appointment of the right length. The form asks for no figures and no documents. It also serves as a tax preparation client intake form for returning clients, because “Your firm” is one of the answers on step three. Practices that keep the books as well can copy it and edit the questions to create a bookkeeping client intake form.

Why this structure converts

  • Filing status is a question every taxpayer can answer immediately.
  • The income step accepts several answers, so a client with wages and rental income is described accurately.
  • No amounts are requested, which means the client can finish without opening a statement.
  • The open notes field comes fourth, after three quick answers.

Make it yours

  • Add a dependents question

Add a step with number buttons for dependents so the subject is covered before the first appointment.

  • Ask for the tax year

If you prepare prior-year returns, add a step asking which year the client needs help with.

  • List documents at the end

Rewrite the thank-you message to name the documents each client should bring, such as wage statements and last year’s return.

Frequently asked questions

What should a tax client intake form ask?

It should ask the filing status, the sources of income and who prepared the previous return. Figures and documents are easier to collect after the client has booked an appointment.

Can clients upload documents through the form?

The template has no upload step. You can add one, although many preparers prefer to collect documents through a secure portal.

Can I use it for bookkeeping clients?

Yes. Replace the filing status step with a question about the type of business, and the form works for bookkeeping inquiries.

Where do new client inquiries go?

New inquiries are emailed to you by default. Growform can also send them to Karbon, Jetpack Workflow, HubSpot or a Google Sheet.
