Event registration form template

This five-step event registration form asks the ticket type, the number of attendees, the preferred sessions and how they heard about the event, then the contact details. You can clone this template into your Growform account and rename the sessions.

What the form asks, step by step

Registration begins with the ticket, which is the decision the attendee has already made. The remaining questions help you plan the rooms and the marketing.

  1. Step 1: “Which ticket would you like?”

General admission, VIP, group and student tickets each need their own confirmation.

  2. Step 2: “How many people are you registering?”

The number of attendees tells you how many places to hold for each booking.

  3. Step 3: “Which sessions interest you most?”

More than one session can be selected, which helps you plan room sizes.

  4. Step 4: “How did you hear about the event?”

This answer shows which invitation or channel filled the most seats.

  5. Step 5: “Who should we send the tickets to?”

Name, email and phone come last.

  6. After submitting

The final screen says a confirmation is on its way, and you can add the venue and the date.

Who it’s for

Conference organizers, training companies, venues and marketing teams can use this template for any event that needs a headcount before the day. Each registration records the ticket type, the size of the party, the sessions that person wants to attend and the channel that brought them. The template does not take payment, so it suits free events, invitation-only events and events where you send an invoice afterward. This event registration form template is easy to adapt, because every ticket and session is an editable button.

For business audiences, see the B2B event and corporate event templates.

Why this structure converts

  • The ticket question comes first because the attendee already knows the answer.
  • Group bookings are captured in one submission, so a team organizer registers everyone at once.
  • The sessions step accepts several answers, which gives you demand figures for every part of the program.
  • The contact step is headed by a question about receiving tickets, which gives a clear reason to complete it.

Make it yours

  • List your own sessions

Replace keynotes, workshops, networking and exhibition with the real session titles so attendees choose from your actual program.

  • Add a company step

For business events, add a text step for the company name so badges can be printed before the day.

  • Confirm by email

Connect the form to your email platform so each person receives a confirmation soon after they register.
